Medical Director of Transitional Care and Clinician Educator

Washington University School of Medicine seeks a full time General Internal Medicine faculty member for the Department of Internal Medicine in St. Louis, MO.

  • Serve as medical director for a transitional care clinic for a large academic medical center
  • Precept Internal Medicine Residents in continuity clinic anchoring a resident interdisciplinary care team

 The General Internal Medicine faculty member will teach excellent, evidence-based, efficient patient care services to the IM residents caring for ambulatory patients in their outpatient practice. They will serve as the medical director for a hospital transitional care clinic, leading a team of social workers, case managers, advance practice providers and physicians to care for recently discharged patients with a focus on social determinants of health. They will lead key strategic initiatives for readmission reduction developed in collaboration with hospital leadership as well as partner with local organization/agencies to improve care coordination.

 

RESPONSIBILITIES:

  • Serve as Medical Director in transitions of care clinic
      • Assist with development of relevant clinical pathways, policies and procedures to optimize care in the clinic
      • Provide leadership and direction in for strategic growth opportunities
      • Partner with organizational leadership to implement key strategic initiatives
      • Participate in review of quality and patient safety data
      • Develop and participate in provider and staff education
      • Partner with community agencies and national organizations to improve care coordination and connect patients with necessary resources
      •  

        • Supervise and teach house staff in the resident outpatient practice
            • Team-based care of residents, nursing anchored by faculty member
            • Co-located support services include community health workers, social work, pharmacy, diabetic educators

           

          • Eligible to obtain a Missouri State medical license
          • Board certification (or board eligibility) in Internal Medicine
